Oh ... this is one of my favorite simple origami model. Aside from personally having fondness for baby turtles, the design for this model is so smooth and precise and makes for a perfect gift!
This model is designed by the Japanese origamist Katsuhisa Yamada. He has published many origami books and has created a lot of adorable models. Check his website here.
